Fill These Out
I would like you to fill out the client information form (one for each partner in a couple), the relevant consent form (individual or couples), and, if applicable, an insurance or Medicare form. You can wait to do this until you come for your first session, but filling them out beforehand will save time so we can talk more fully when we meet. You will need the free Adobe Reader to open the forms.
